Attorney and Supervisor Forms

We welcome your interest in having Harvard Law students work on a clinical or pro bono basis at your law firm or organization. Forms can be filled in and emailed as attachments, or printed out and mailed to us. If you have any questions, please contact us.

 Getting Started

Please complete the New Organization  Form if you would like to register with our program as a supervisor and have law students work with you on legal projects. For any open positions, please complete the New Position Form.  All supervisors should review the Clinical Supervisor Handbook for information on how to supervise HLS clinical students.

 Clinical Supervisor Evaluations

Externship supervisors should use the mid-semester supervisor evaluation to evaluate clinical students halfway through the Fall and Spring semesters (winter clinicals use Final evaluation only). Complete the final supervisor evaluation to evaluate clinical students at the end of the Fall, Winter, and Spring semesters.
Independent Clinical supervisors should use the mid-semester supervisor evaluation to evaluate clinical students halfway through the Fall and Spring semesters (winter clinicals use Final evaluation only). Complete the final supervisor evaluation to evaluate clinical students at the end of the Fall, Winter, and Spring semesters.

 Pro Bono Forms

Supervisors should review and agree to the Project Terms and Conditions prior to any pro bono placements. After the student's placement has ended, the Time Log and Supervisor Evaluation should be submitted to the Office of Clinical and Pro Bono Programs within 10 days.
