SFTP Account Request Form for Student Organizations

This form is only for student organizations. If you are requesting an SFTP account for another purpose (e.g., a course, faculty, program, or departmental site), please flll out this form instead.

NOTE: required fields are prefixed with an asterisk (*)

*Name:

The name of the user whom you wish to be able to edit your organization website.

*HLS affiliation:

This user is already affiliated with HLS (e.g., student) and has a username. This is an external user (e.g., a consultant) who does not have an HLS username.

*Email address:

Their complete email address, e.g., jharvard@law.harvard.edu. If they are already affiliated with HLS, please provide their HLS email address or username.

*Phone number:

The phone number at which the user needing access can be reached. This is required in order to give external users their password.

*URL or description:

If the website exists already, paste the "home page" URL here. For new sites, please enter a brief title or description.
