We are requiring this form because your son/daughter indicated that you are currently REQUIRED to make loan payments on education debt that was incurred by you to pay for the education-related expenses of yourself, the HLS Student, or any other member of your primary household (i.e. the HLS student's siblings). The information you provide may make you eligible to receive an income protection allowance against your income for these payments as part of our determination of your son/daughter's eligibility for need-based grant assistance. If you are not currently required to make loan payments on education debt, please contact our office so we can adjust your application requirements.
What follows are the STRICT guidelines of the types of loan payments that qualify AND the types of specific paper documentation which it is necessary for you to give us in order for us to be able to take these payments into account in our determination of your son/daughter's aid eligibility.

In some cases, in order to receive an allowance for parent education loan payments, you will be required to submit this web form AND to submit actual paper documentation to our office to verify the data you are providing on this web form.
If we determine (on a case by case basis) that you need to submit additional documentation other than this web form, someone from the SFS staff will contact you directly to make a formal request. In cases where a request for additional documentation is made by SFS, please note that no allowances will be given until proper documentation is received.

When submitting this documentation, be sure that to use our document cover sheet found on the forms page of our website so we can properly identify the documentation as yours and have it route correctly to your son/daughter's electronic file.
NOTE FOR STUDENTS WHOSE BIOLOGICAL PARENTS ARE DIVORCED OR SEPARATED: We ask that each parent submit this form separately for their own individual education loans, if applicable. In addition, each parent's household should submit the form for all educational loans they are required to make payments for, even if the loans are held jointly with a new spouse/step-parent or if the loan is held individually by a new spouse/step-parent.
