Disability Accommodation Request Form

Harvard University is required by Section 504 of the Rehabilitation Act and The Americans with Disabilities Act (ADA) to provide effective auxiliary aids and services for those with documented disabilities if such aids are needed to provide equitable access to Harvard's programs and services. Both the ADA and Section 504 protect individuals from discrimination and ensure reasonable accommodations.

To establish that an individual is covered under the ADA, documentation must indicate that the disability substantially limits one or more major life activity, and supports the request for services, accommodations, academic adjustments, and/or auxiliary aids.

If you have a digital record of your medical documentation, you may e-mail it directly to disabilityservices@law.harvard.edu. If you do not have a digital record of your medical documentation, you may bring it to the Dean of Students Office, Caspersen 3039, or fax it to (617) 496-0544.
Additionally, documentation may be mailed to:

1585 Massachusetts Ave. Suite 3039
